First you have to see how she is with her near sight vision and you can do this by letting her identify some shapes. For the distance vision you have to let her identify letters of different sizes. The duochrome test is designed to see if she can differentiate green from red. She also needs an ophthalmoscopy and a computerized eye test so you can see exactly what the problems is with her vision. For the manual eye test take lenses of different diopters and see which one of them suits each one of her eyes.
